What is transanal irrigation?

Transanal irrigation (TAI), sometimes also called bowel irrigation, is a minimally invasive treatment for bowel dysfunction which uses lukewarm water to irrigate the rectum and colon via the anus in order to initate peristalsis (bowel movements) and expel stool.

Regular use of TAI prevents bowel leakage and accelerates transit through the colon. With TAI the rectum and distal colon will be efficiently emptied of faeces and it takes approximately 2 days until new faeces reach the rectum. This allows the user a period free from accidents and increased independence. The therapy is usually well tolerated and has low incidence of major side effects.

There are many different underlying causes which could cause these symptoms and benefit from treatment with bowel irrigation. The most common ones include:

  • Neurogenic Bowel Disorder

    Neurogenic Bowel Disorder (NBD) is the term used to describe dysfunction of the colon due to disruption of nerve signals. This can lead to disrupted peristalsis and interruption to urge signals. 

    Examples of conditions that may be affected by NBDs are:

    • Spinal cord injury; traumatic or disease acquired
    • Parkinson’s disease
    • Multiple Sclerosis
    • After stroke

  • Functional Bowel Disorder

    Functional Bowel Disorders (FBD) are bowel disorders without any morphological, metabolic or neurologic cause. There are other definitions in the literature but generally  FBD is diagnosed after full investigations which fail to identify an underlying organic cause.

    Examples of FBDs are:

    • Chronic constipation (or functional constipation)
    • Faecal incontinence
    • Slow transit constipation (STC)

  • Other organic causes of bowel dysfunction

    Organic causes may vary, but can include: 

    • Congenital conditions like Hirschsprung disease and anorectal malformations
    • Matabolic disorders
    • Endocrine disorcer
    • Pelvic Floor Dysfunction (PFD), e.g. rectal damage after childbirth and prolapse.
    • Post-surgical situations, e.g. LARS.
    • Medication – overuse/underuse of bowel medications, but also side effects of medication for other conditions. ‘Over-the-counter'/non-prescribed medication can also affect bowel function.
